* Basic Visits are $50 for the first 30 minutes.
* Longer visits will be charged at $20 per 15 minutes beyond the first half hour.
* Supplies and tests are additional.
* Visits to locations outside of Boulder (for example, Lafayette, Lousville, Nederland, etc) are an additional $20 to $30, depending on distance.
* A receipt may be provided for you to submit to your insurance provider.
* I am unable to accept insurance and request payment at the time of service, but please discuss any financial hardship with me.
* Phone consultations are free for the first 10 minutes. Calls are welcome for further information regarding my practice.
